New Patient Registration

To join the Practice please call at the practice to book an appointment and collect Form GMS1 (Family Doctor Services Registration) to complete and bring along to your first appointment. At this appointment you will also need to bring evidence of residency at your address (Bank / Utility statement etc) along with photo ID.

You will receive a health check with the healthcare assistant and will be asked to complete a questionnaire on your personal history to assist in giving you treatment until your records are received from your previous doctor.

When all the above is undertaken you will be registered with the practice.

Please note that for all children under the age of 5 we will also require a copy of the child’s immunisation records. We also require immunisation records for all non UK patients under the age of 16.
